What Matters to You Day

It’s a simple question that can have a big impact…

On, June 6th join us in asking What Matters to You?

WMTY campaign is a worldwide movement putting the patient voice at the center of care, encouraging and supporting meaningful conversations between the patients and families who receive healthcare, and the staff, physicians and volunteers who provide it.

Taking part is easy! It includes three simple steps:

1)    Ask what matters… we encourage you to embed this question into your practice each and every day.

Start by having conversations about what matters…

Healthcare Providers

Ask your patients and families “What are the things that are important to you at the moment?” or “What can I do to best support you in your care today?”

Patients & Families

Reflect on what matters to you and share it with your healthcare team.

2)    Listen to what matters…

Listen with the intent of gaining a better understanding of what is important to patients and families. With this crucial insight, we are in a much better position to partner with patients in their care journey.

3) Do what matters…

Providing care that focuses on the needs of patients and families leads to stronger partnerships, improved treatment and health outcomes, patient safety, and an overall better care experience.

Check out the display and share “What Matters to You? throughout the day in the main gather space at ACH!

ACH Leader Rounding Relaunch at ACH

The Patient & Family Centred Care team, in partnership with executive leadership, is excited to announce the successful relaunch of Leader Rounding at ACH! Speaking with patients and families helps leaders gain valuable insights as to how patients and families are experiencing their care at ACH and recognize staff who have supported families. Leader Rounding is an opportunity for leaders to connect with patients, families, and staff on a regular basis to identify improvement opportunities and recognize staff achievements.

With the support of executive leadership, the first session was held on April 23rd with a record number of 18 leaders in attendance. Feedback provided by leaders expressed the value of insights gained from patients and families during this practice.

Check out the AHS My Care Conversation App

Research also shows that patients and families may forget up to 80% of health care information discussed at clinic visits.

The AHS My Care Conversations app is a smart phone application that allows patients and families to record their conversations with their health-care providers to help reduce stress and assist with recall and understanding. Being able to record and re-listen to their appointments helps patients review and understand the appointment's discussion points and clarify any misunderstandings. Patients can also choose to share their recordings with trusted individuals who were not able to attend the clinical visit with their care providers.

The AHS My Care Conversation app is available on the app store (for Apple devices) and on the Google Play store (for Android devices). It is a free download. It includes a built-in tutorial that will walk users through the key functions of the app.

The app will remind users of important things like remembering to tell your health care provider that you would like to make a recording of the appointment, and to not record in public spaces. The clinician may request a copy of the recording. Users do not need the clinician’s consent to record.
