What does The Human Connection mean to you?

Bring together an ACH team and share with us what it means to you by decorating a puzzle piece. We supply the puzzle piece and you provide the decorating supplies.
All decorated puzzle pieces will be displayed in the cafeteria during Patient and Family Centred Care Week (Nov 4-8th). Note: Only the front side of the puzzle piece will be displayed.
The winner will be announced at the PFCC Week celebration event at noon on November 8th in the Amphitheatre. Please come out for this event. Everyone is welcome.
The winning team will receive a trophy and a gift card for some treats.
To receive a puzzle piece for your team - email us at achpfcc@ahs.ca or bring this completed form to the FCRC (second floor). Note: there is a limited supply of puzzle pieces, so first come first serve.
All decorated puzzle pieces and this form must be returned to the FCRC by October 30th, 2019
Additional Challenge Information:
All puzzle pieces will be displayed during Patient and Family Centred Care Week in the cafeteria. All patients, families/caregivers/staff/physicians and volunteers will be given the opportunity to vote for their favorite during the week.
A judging panel will judge the puzzle piece submissions on Thursday, November 7th. The entries will be judged based on conceptualization, connection to The Human Connection theme, appearance, originality, and team involvement. The winner will be selected based on 50% voters favorite and 50% judging panel selection.
The winner will be announced at the PFCC Week celebration event on Friday, November 8th from 12:00 ‐ 1:00 PM in the Amphitheatre (Presentations at 12:15 PM). Everyone is welcome to attend. Please send at least one representative from your team to this event for the award presentation.
The winning team will receive the Human Connection Team Challenge trophy and a gift card to buy some treats for the team.
