Care is a Family Affair
Team Challenge

What does Care is a Family Affair/Family Presence mean to you?

Bring together an ACH team and share with us what it means to you by decorating and filling an 8 x 10” wooden picture frame. We supply the frames and you provide the decorating supplies.

All frames will be displayed in the cafeteria during Patient and Family Centred Care Week (Nov 5-9^th, 2018). Note: Only the front side of the frame will be displayed.

The winner will be announced at the PFCC Week celebration event at noon on November 9th in the Amphitheatre. Please come out for this event. Everyone is welcome.

To receive a frame for your team - email us at achpfcc@ahs.ca by October 25^th, 2018 with the following information:

Note: there are a limited supply of frames, so first come first serve.

All decorated frames must be returned to the FCRC by end of day Wednesday, October 31^st, 2018

Additional Challenge Information:

All frames will be displayed during Patient and Family Centred Care Week in the cafeteria. Staff, patients, and families will be given the opportunity to vote for their favorite.

A judging panel of staff and patients and families will judge the entries on Thursday, November 8^th, 2018. The entries will be judged based on conceptualization, connection to compassion, appearance, originality, and team involvement.

The winner will be selected based on 50% voters favorite and 50% judging panel selection.

The winner will be announced at the PFCC Week Celebration on Friday, November 9^th, 2018 from Noon to 1:00 PM in the auditorium, presentations at 12:15 PM. Everyone is welcome. Please send at least one representative from your team to this event for the award presentation.

The winning team will receive the Care is a Family Affair Challenge trophy and a gift card for some "treats".
